On 4/19/23 the following files were reviewed: Child 1: Missing doctor and dentist information, health history blank, last dental and medical exam dates missing. Child 2: There wasn't a child file available with WAC items mentioned above for the licensor to review for child 2 who was present that day. Additionally, there was not a CIS record for this child. Child 3: No pick up person listed, if "none" write "none." No dentist listed who name of dentist they prefer. Additionally, there was not a CIS record on file.
